Abstract:
In Taiwan, many medical institutes have successfully transformed themselves into
filmless medical environments. At the same time they also equip themselves with the
prerequisite to implement paperless electronic medical record by being filmless. In
this article, concept of structured report will be raised to consolidate the possibility of
bridging from filmless to paperless. Furthermore through the viewpoints of public
health and knowledge management, the importance of structured report will be again
highlighted.

摘要:
在台湾历经无片环境洗礼的医院与医学中心, 正为本身准备好迈向全电子病历
的绝佳条件, 如何更进一步进化到无纸化的医疗环境是极艰难的挑战. 本文将透
过介绍 DICOM 结构性报告点出其中的可能性, 再引申到以公卫角度与知识管
理来强调DICOM 结构性报告的重要性.

结构性报告DICOM背景:
在放射医学的内涵中, 影像与报告是一体两面, 具有相同的重要性.
DICOM [1] 是数位医学影像格式与传输的标准, 自公元2000年发表全新资讯物
件定义 ( I.O.D.: information object definition ) 结构性报告 Structured report
( modality type : SR ) 後, 就宣告了放射影像报告格式统一的可能性.
过去旧式的报告系统在设计上, 大部分是以所谓的 plain text 作为储存报告的格
式与内容, 而产生的过程也需历经很多程序与时间, 如录音, 打字, 审核, 审退,
确认等等步骤. 有任何一个步骤出问题, 报告的传播 ( distribution ) 与正确性便
会受到影响与质疑, 而且病人住院天数往往会因为报告的延迟而不自觉的拉长,
造成医疗成本的增加, 与医疗品质的降低, 而後者常是因为冗长的报告内容造成
临床医师实际阅读与判断的困难, 而且报告内容与标准用词的不一致性也是影
响临床医师逻辑思考的重大因素.
以 Plain Text 为影像报告的弱点除了上述医院营运与医师临床工作上的问题外,
对於医学研究与公卫领域的影响也极大. 全文检索系统 ( full text se
